If the “Add Document” or “Edit Document” menus appear to overlap each other the issue may be with your browsers zoom/scale settings.

You can resolve this by resetting your zoom/scale settings to 100%

We have noticed Google Chrome looks like the following when set to 90%:

You can set your browser scaling in the browser settings. 

Or 

You can set the zoom/scale options to 100% use the following keyboard shortcuts 
Windows: Control+0
